As more autism reported, doctors say check early

CHICAGO (AP) ? At 18 months, Cristina Astacio spoke only a few words, wouldn't respond to her name and shunned other kids in her day care group. Last October, her worried parents found out why.

She has a mild form of autism, a diagnosis being given to more U.S. children than ever before, largely because of more awareness and better diagnosis.

According to new government statistics, the rate is about 1 in 88. That means autism is nearly twice as common as the government reported just five years ago. The largest increases are in Hispanic kids like Cristina.

The definition of autism has changed over the years, and Cristina might not have been considered autistic two decades ago.

But experts say kids like her are lucky in a way, because her parents recognized early that something was wrong. The Centers for Disease Control and Prevention report issued Thursday found that 40 percent of kids weren't diagnosed until after age 4. Evidence shows that children who are identified early and get help have the best chance for reaching their potential, said CDC Director Dr. Thomas Frieden.

The American Academy of Pediatrics recommends autism screening for all children at age 18 months and 2 years.

Diagnosing the developmental disorder relies on observing behavior. Autism can't be cured, but treatment including intensive behavior therapy can help many kids function better.

The academy's Dr. Susan Hyman said many children who aren't making eye contact and aren't talking "may have autism, but they may have other things." She said it's important for parents to be persistent about their concerns with their doctor so their kids can be evaluated.

Kristy Batesole, of Atascadero, Calif., says she suspected something was wrong with her son, Keegan, even when he was a hard-to-calm overly fussy baby. He learned words, but by age 2 stopped talking, would spend hours opening and closing doors and sometimes bang his head on the ground.

Though he started getting special help in preschool in Nevada, he wasn't formally diagnosed with autism until last year, at age 6, after the family moved to California, where there are more autism specialists.

Cristina Astacio gets two hours of behavior therapy six days a week. Her mom, Charisse, says the little girl now responds to commands and speaks about 50 words. The most special are two words Cristina never said before. "Now she says 'mommy' and 'daddy,'" Astacio said. "It's wonderful."

Christina's dad, Christopher, is a special-education teacher in the New York City; most of the kids in his class are Hispanic and many have autism.

"I remember back in the past, a few kids here and there had autism, not like the way it is now," Astacio said. "I'm really curious why so many kids are being diagnosed."

Experts, including CDC researchers, think broader screening and better diagnosis have largely contributed to that. But autism's cause remains a mystery, and government researchers are seeking answers.

The latest numbers are based on 2008 data from 14 states and suggest that autism rates have climbed 23 percent since 2006. Rates are highest in boys and white children. But the biggest rate increase was among Hispanics, from 1 in 270 in 2002 to about 1 in 125 in 2008.

That rate increase also is likely due to better awareness. The CDC report says there's no strong evidence of any racial or ethnic difference in risk factors for autism and that it's likely the condition is underdiagnosed in blacks and Hispanics.

Melissa Miller, a St. Petersburg, Fla., mom whose daughter, Chelsea, was diagnosed last year at age 2, said many people still misunderstand the disorder.

"I think many people hear 'autism' and think 'Rain Man,'" she said, referring to the 1988 movie featuring Dustin Hoffman as the mathematically brilliant but socially impaired autistic savant.

"The autism spectrum is so vast, and all of our children are different. Many of them don't rock back and forth or have savant skills. They are sweet, affectionate, intelligent, goofy ? and exhausting ? kids," Miller said.

Proposed revisions in the manual that doctors use to diagnose mental illness would streamline autism criteria. Critics contend the suggested changes would be too narrow and exclude children who need educational and behavioral services.

Hyman noted that since the manual's last revision, in 1994, much has been learned about autism. "There's a real possibility the new definition will be better for children," she said Thursday at a CDC news conference.

CDC officials say research into causes of autism will help determine if there's been a true increase or just better diagnosis.

Genetics is believed to play a role. Studies have found no connection with childhood vaccines, but other factors under investigation include mothers' illnesses or medication during pregnancy. First results from the CDC study are expected next year.

Geraldine Dawson, chief science officer for the advocacy group Autism Speaks, said the new figures indicate "a public health emergency that demands immediate attention."

Her group estimates that U.S. autism costs total $126 billion each year, including costs related to diagnosis and treatment. That estimate also includes treatment for severely affected adults and lost wages.

>> it is a favorite toy of newt gingrich and rick santorum . this is the kind of thing it is easy to say poor mitt romney . he doesn't really get a break and he didn't say it himself. it was an aide. and it is not really so fair that the media jumped on it and it became this epic gaffe. but truly, we all know a year from now if mitt romney is lost, people will call him etch a sketch for the rest of his political career. it is the kind of thing that sticks and unfortunately, when those things stick, you're stuck with them. so i think rick santorum and newt gingrich should leave the etch a sketches in the car and finish up this joke. i think mitt romney is stuck with that joke forever.

Virgin Mobile USA's text and data services are out across the USA

As customers of Sprint's contract-averse MVNO have probably noticed, Virgin Mobile has been having a national outage of its text and data services for at least several hours now. While we're hearing from some customers it's been out all day, acknowledgement of the disruption only just became Facebook official. There's no word yet when it will be resolved however, as we're only getting the "engineers are working to resolve it" boilerplate for now. Until then we'd suggest enjoying the few activities that are still possible without wireless internet access, if those are even still a thing.

'Stand Your Ground' law muddies Martin case

The Department of Justice has launched an investigation into the death of the teen shot in the picturesque city of Sanford, Fla. NBC's Ron Allen reports.

Florida?s 2005 ?Stand Your Ground? law, which says a citizen doesn?t have to retreat before using deadly force against an attacker, could throw a legal wrinkle into the case of a neighborhood watch captain who shot to death an unarmed black teenager.

Police in the central Florida town of Sanford have said that 28-year-old George Zimmerman says?he shot 17-year-old Trayvon Martin in self-defense during a confrontation in a gated community. Police have described Zimmerman as white; his family says he is Hispanic and not racist.

Police did not arrest Zimmerman after the shooting, but State Attorney Norm Wolfinger announced Tuesday the case will go before a Seminole County grand jury.

Legal experts, gun-rights advocates and gun-control groups contacted by msnbc.com offered varying opinions on whether Zimmerman can avoid criminal charges under Stand Your Ground.

Richard Hornsby, an Orlando-based criminal defense attorney, says he thinks the grand jury is likely to indict Zimmerman for manslaughter, a second-degree felony punishable by up to 15 years in prison. Less likely is a more serious charge of second-degree murder, a crime that implies intent and that is punishable by up to life in prison, he said.

?This case isn?t even a close call to me. This is a case of a guy trying to be a vigilante,? Hornsby said. ?It wasn?t like he was trying to avoid trouble. He brought a firearm to a fistfight.?

?My gut feeling he will ultimately be charged with some type of manslaughter charge,? agreed Philip Sweeting, retired deputy police chief for the Boca Raton Police Department and a law enforcement consultant who has testified? in court cases as an expert in police shootings and use of force.

?My gut reaction was this was an accidental discharge,? Sweeting added. ?If you put yourself in the shooter?s position and you?re wrestling with this kid and a gun goes off, what are you going to tell the cops??

If indicted, Zimmerman can raise the Stand Your Ground defense under?the 2005 law signed by Gov. Jeb Bush. That legislation, derisively called the ?Shoot First? law by its critics, gives Floridians the right to use deadly force to defend themselves in public places without first trying to escape. The National Rifle Association lobbied hard for the bill, saying it would allow citizens to better protect themselves from violent crime.

The key section of the law states:

A person who is not engaged in an unlawful activity and who is attacked in any other place where he or she has a right to be has no duty to retreat and has the right to stand his or her ground and meet force with force, including deadly force if he or she reasonably believes it is necessary to do so to prevent death or great bodily harm to himself or herself or another or to prevent the commission of a forcible felony."

Kendall Coffey talks about the legal developments in the Trayvon Martin case and Florida's "Stand Your Ground" law.

Hornsby says the 2005 bill allows Zimmerman?s lawyers to argue to a judge ? before any trial -- that the case should be dismissed on grounds he was permitted to defend himself. If the case isn?t dismissed,?they could argue self-defense at trial.

Daniel Vice, senior attorney with the gun-control group Brady Center to Prevent Gun Violence,?contends ?Stand Your Ground,? combined with Florida laws allowing people to carry guns in public, have made it difficult to pursue criminal charges against people who shoot others and then say?it was self-defense.

?All you have to say is that you reasonably believed you were threatened, and the only person who can dispute that is the person you have just killed,? Vice says.

"It's very hard to bring these types of cases because the 'Shoot First' law combined with public carrying of loaded guns protects people who engage someone and shoot to kill."

According to a 2010 review by the St. Petersburg Times, reports of justifiable homicide tripled after the law went into effect. It has been invoked in at least 93 cases with 65 deaths, used to excuse violence in deadly neighbor arguments, bar fights, road rage and even a gang shootout, the newspaper reported.

Dynamically shot and paced like a thriller, the film has the density and moral prickliness of a good novel.

These people seem so real they might live next door. And they probably do.

Very few movies capture as convincingly as A Separation does the ways in which seemingly honorable decisions can lead to interpersonal conflict -- even disaster.

To say the piercing Iranian film A Separation is about divorce is a bit like saying The Wizard of Oz is about a pair of slippers.

"A Separation" moves beyond one couple's sundering marriage to reveal growing rifts between generations, ideologies, religious mind-sets, genders and classes in contemporary Iran.

"A Separation" is a great movie, a look inside a world so foreign that it might as well be another planet, yet so universal that its observations are painfully familiar to anyone, anywhere.

A clever, insightful and heartfelt examination of human frailty that makes watching subtitles seem like a breeze.

It's fast paced, exciting, thrilling, edgy, moving, engaging, and - in its portrait of a justice system almost radically alien to the one I live under - absolutely fascinating.

Although the film might serve as a portrait of Iran's two conflicting social groups as reflected in the Green movement/Ahmadinejad clashes, it is much more about moral contradictions that any society has to face. A masterpiece.

It's the little moments in Farhadi's film that are its most important, speaking every bit as loudly as its big, narrative-driving moments.

If any one film can re-inject life into an entire national cinema, it's A Separation.

You may also find that some aspects of this very foreign story seem disturbingly not all that foreign.

An outsider could see these characters as misguided. That's not the reality however. Farhadi slips us into their shoes and we appreciate each of their perspectives.

Asghar Farhadi has written a superb screenplay and directs it with equal brilliance. He has managed to make a film that says something about the state of affairs in his Iran and weaves that message into an engrossing tapestry of mystery and drama.

The complex realities of human emotions and experience are more than sufficient to carry a film, and Farhadi doesn't let fussy direction or attempts to be overly clever get in the way of letting this brilliant, quietly insightful story unfold.

A clarion cry that cuts through the xenophobic clutter and the dense fog of war to show that not everyone "over there" is a boogeyman waiting to jump out of the closet. If that sounds terribly simplistic, just consider where we live.

A Separation is more than a character study; it's a deftly plotted drama that unfolds layer after layer and will leave you asking all kinds of questions about your beliefs and moral judgments. Fascinating stuff.

The story has no winners, and the daringly ambiguous ending defies simple conclusions. Even the title can be taken multiple ways.

The movie opens the door on a foreign culture, yet the people do not seem that dissimilar. .... They deal with some of the same struggles ... -- generational and class conflicts, ideological differences, and connecting and communicating emotionally.

A devastating portrait of two families in crisis, with their class differences representing the title's true 'separation.'

"A Separation" deals with some very murky issues, but it does so with astonishing clarity.

The movie is a singular mix of the foreign and the familiar, but is it the best 2011 film in the whole wide world? A 99? I can't say that I concur with my colleagues on that score.

A Separation explores how presumed social norms are extremely tenuous and how threats to these almost illusory ideas can threaten our sense of personal security.

Farhadi does not talk down to his viewer, and he trusts his audience to make their own decisions.

Old Havana restaurants quietly going smoke-free

HAVANA (AP) ? It's a quintessentially Cuban experience: Capping off a meal with a snifter of rum and an aromatic cigar.

This Caribbean nation is renowned around the world for its pungent Cohibas, Montecristos and Romeo y Julietas, but on the island, stogie-lovers are increasingly being told to take it outside.

A number of state-owned restaurants in Havana's picturesque colonial quarter have quietly gone smoke-free indoors in recent weeks as authorities there enforce a 2005 measure that has been almost universally flouted across the country.

The goal is to improve the culinary experience and safeguard the health of both diners and employees, but it's also raising eyebrows among cigar aficionados and cigarette smokers who say the right to light up is part of the tropical country's charm. Already, public smoking bans have spread to cities worldwide, from New York to Beijing.

"No-smoking areas? It's incredible!" said Michael Kuntze, a 59-year-old German day care manager who was savoring a long cigar and sipping rum and cola in the Hotel Conde de Villanueva, home to one of the city's most popular cigar rooms.

Kuntze and six other smokers from Hamburg were on a nine-day tobacco tour, sampling more than three cigars daily and selecting 50 each to bring home.

"That (no-smoking ordinances) is what we have in Europe, in Germany, but we don't want this here," he said, as aromatic smoke rose from the thick ash at the end of his stogie. "This is why we are here. Not to sit inside a small smoking lounge, no. Never."

Officials say the Conde de Villanueva, a favorite of cigar tourists like Kuntze, will continue to let guests and diners smoke.

At least nine state-run restaurants in the small, tourist-packed colonial area of Havana have banned smoking inside since the end of 2011, and more will do so in the near future, said Tannya Sibori, publicity manager for Habaguanex, the state-run business that administers tourist concerns in Old Havana.

Only sealed, air-conditioned dining rooms are affected, and Habaguanex restaurants all still have open-air spaces for smokers. There is no word on a ban for bars or nightclubs, and the owner of one of Havana's private restaurants said he had received no guidance on whether the "paladares" must follow suit.

Habaguanex officials described it as a bottom-up trend rather than an order from on high, but said it's possible there could be an official decree at some point.

"There is a campaign at the world level in which we should also take part, where we are helping to create healthier spaces even for the smokers themselves," Sibori said.

Diners are still welcome to enjoy their after-dinner cigar ? just move to the outside tables, please, where you can people-watch on the quaint, cobblestone plazas and enjoy the balmy, tropical breeze.

"Cuba has an eternal summer, and you can take advantage of the terraces and exterior spaces," Sibori said.

Still, such a thought is anathema to some in Cuba.

The island has an ingrained tobacco culture and is proud of its world-famous cigar industry, which brought in $401 million in sales last year. When other goods were in scarce supply, cigarettes continued for years to be part of islanders' monthly ration books.

Even nonsmokers cherished the subsidized tobacco, which they sold on the black market or traded for other goods.

But Fidel Castro gave up his trademark Cohibas in 1985 on doctors' orders, and authorities have gently discouraged the vice.

Today, state-run radio airs public service announcements about the benefits of quitting, and cigarette packs carry health warnings. Cigarette dispensing machines have disappeared. Authorities began phasing out the tobacco ration in the 1990s and fully eliminated it in 2010.

Castro even joked about the ills of smoking cigars, saying "the best thing to do is give them to your enemy."

Still, government numbers say as many as four of every 10 Cubans smoke ? though that's way down from estimates of 60 to 70 percent in the 1970s.

Cuba has had a resolution on the books since 2005 banning smoking in theaters, stores, buses, taxis, restaurants and other enclosed public areas.

But many are unaware of the law, and it's common to see people brazenly light up practically anywhere they theoretically shouldn't: offices, stairwells, elevators, buses, trains. Farmers rise early to tend their crops with cigars dangling from the lip, and even elderly women meander through crumbling city streets chomping on soggy cigar stubs.

"This is a country of smokers, a country with an important tobacco tradition," Sibori said. "Changing habits that form part of our roots is very difficult. I imagine that little by little things will be implemented so that this takes shape, but I still think there is much work to be done."

The Habaguanex initiative is also being carried out with zero fanfare, and no announcement in state-run media.

On a recent afternoon in El Mercurio cafe, tables were packed with French tourists chowing down on pork loin, rice, beans and fried plantains. Missing was the stale, smoky haze that used to hang over the tables.

Waiters said they appreciate going home at night in clothing that doesn't smell like an ashtray. Chefs take smoke breaks outside the back door without apparent complaint. There's still some clandestine indoor smoking at some places late at night when most clients are gone, however.

"Why not?" said Thomas Gabrisch, a Dusseldorf, Germany, music professor who was puffing on a slender cigarillo outside El Mercurio, when asked if he was OK with the indoor ban. "I think (smoking) bothers a lot of people. For me it would not be a problem ... But I think a lot of people would like to stay inside."

That sentiment was echoed by Dirk Brodersen, one of the aficionados from Hamburg, who said it was an adventure to be able to smoke the previous night at a basement music club.

"What is Cuba? Rum, cigars, sun and people," Brodersen said. "Cuban jazz without a cigar ? not so good."

'Deforce: America's Past. America's Future. Detroit's Present': New Film Examines City's History (VIDEO)

"You can't fight a problem until you understand it's roots."

That's the challenge undertaken by the filmmakers behind "Deforce: America's Past. America's Future. Detroit's Present," a new documentary examining Detroit's political history and the current state of the city.

The movie opens Wednesday evening at the Charles H. Wright Museum of African American History. It's shown at film festivals across the country and has garnered praise from the likes of legendary documentarian Ken Burns, who called the it "an essential study of an iconic industrial aftermath."

For the Detroit premiere, the filmmakers and some of their interviewees, including residents, community leaders, academics and elected officials, will come to the Wright to discuss the film.

The project was initiated, co-written and produced by metro Detroiter Andrew Rodney. He brought New York University Tisch School of the Arts graduate and former Wylie E. Groves High School classmate Daniel Falconer onto the film as director, co-writer and producer.

Rodney and Falconer started by reading up on Detroit's history, they said on WDET's "The Craig Fahle Show."

"You pick what you don't think you've heard in the dialogue that the reading tells you is really critical, but somehow gets left out of the conversation," Rodney said.

"Deforce" tackles Detroit's problems over the last half-century, recording how they've shaped the city and delving into the nitty-gritty of policy decisions about public housing, the effect of the war on drugs, violence and blight -- all while still showing a city full of proud Detroiters.

"We're all on the same side here," says a voiceover in the film's trailer. "This is not downtown versus the neighborhoods, this is Detroiters against a negative future."

"Deforce: America's Past. America's Future. Detroit's Present" makes its Detroit premiere with a screening and panel discussion at 7 p.m. on Wednesday, March 14 at the GM Theater at the Charles H. NEW YORK, March 14, 2012 A new study by researchers at NYU Langone Medical Center reveals that the presence of Helicobacter pylori (H. pylori) bacteria is associated with elevated levels of glycosylated hemoglobin (HbA1c), an important biomarker for blood glucose levels and diabetes. The association was even stronger in obese individuals with a higher Body Mass Index (BMI). The results, which suggest the bacteria may play a role in the development of diabetes in adults, are available online in The Journal of Infectious Diseases.

There have been several studies evaluating the effect of the presence of H. pylori on diabetes outcomes, but this is the first to examine the effect on HbA1c, an important, objective biomarker for long-term blood sugar levels, explained Yu Chen, PhD, MPH, associate professor of epidemiology at NYU School of Medicine, part of NYU Langone Medical Center.

"The prevalence of obesity and diabetes is growing at a rapid rate, so the more we know about what factors impact these conditions, the better chance we have for doing something about it," Dr. Chen said. Looking at the effects of H. pylori on HbA1c, and whether the association differs according to BMI status, provided what could be a key piece of information for future treatment of diabetes, she explained.

Type II diabetes causes an estimated 3.8 million adult deaths globally. There have been conflicting reports about the association between H. pylori infection and type II diabetes. To better understand the relationship between H. pylori and the disease, Dr. Chen and Martin J. Blaser, MD, the Frederick H. King Professor of Internal Medicine and professor of microbiology, analyzed data from participants in two National Health and Nutrition Surveys (NHANES III and NHANES 1999-2000) to assess the association between H. pylori and levels of HbA1c.

"Obesity is an established risk factor for diabetes and it is known that high BMI is associated with elevated HbA1c. Separately, the presence of H. pylori is also associated with elevated HbA1c," said Dr. Blaser, who has studied the bacteria for more than 20 years. H. pylori lives in the mucous layer lining the stomach where it persists for decades. It is acquired usually before the age of 10, and is transmitted mainly in families. Dr. Blaser's previous studies have confirmed the bacterium's link to stomach cancer and elucidated genes associated with its virulence, particularly a gene called cagA.

Regarding H. pylori's association with elevated HbA1c, Drs. Chen and Blaser believe the bacterium may affect the levels of two stomach hormones that help regulate blood glucose, and they suggest that eradicating H. pylori using antibiotics in some older obese individuals could be beneficial.

More research will be needed to evaluate the health effects of H. pylori and its eradication among different age groups and in relation to obesity status, the authors noted.

"If future studies confirm our finding, it may be beneficial for individuals at risk for diabetes to be tested for the presence of H. pylori and, depending on the individual's risk factor profile" Dr. Chen.

In an accompanying editorial in The Journal of Infectious Diseases, Dani Cohen, PhD, of Tel Aviv University in Israel, pointed out that while previous studies have addressed the association between type II diabetes and H. pylori in small samples, this study analyzed two independent large national samples of the general population. Dr. Cohen agreed with the study authors, suggesting that adults infected with H. pylori with higher BMI levels, even if asymptomatic, may need anti-H. pylori therapy to control or prevent type II diabetes. If the study findings are confirmed, Dr. Cohen wrote, they "could have important clinical and public health implications."

NEW YORK, March 14, 2012 A new study by researchers at NYU Langone Medical Center reveals that the presence of Helicobacter pylori (H. pylori) bacteria is associated with elevated levels of glycosylated hemoglobin (HbA1c), an important biomarker for blood glucose levels and diabetes. The association was even stronger in obese individuals with a higher Body Mass Index (BMI). The results, which suggest the bacteria may play a role in the development of diabetes in adults, are available online in The Journal of Infectious Diseases.

There have been several studies evaluating the effect of the presence of H. pylori on diabetes outcomes, but this is the first to examine the effect on HbA1c, an important, objective biomarker for long-term blood sugar levels, explained Yu Chen, PhD, MPH, associate professor of epidemiology at NYU School of Medicine, part of NYU Langone Medical Center.

"The prevalence of obesity and diabetes is growing at a rapid rate, so the more we know about what factors impact these conditions, the better chance we have for doing something about it," Dr. Chen said. Looking at the effects of H. pylori on HbA1c, and whether the association differs according to BMI status, provided what could be a key piece of information for future treatment of diabetes, she explained.

Type II diabetes causes an estimated 3.8 million adult deaths globally. There have been conflicting reports about the association between H. pylori infection and type II diabetes. To better understand the relationship between H. pylori and the disease, Dr. Chen and Martin J. Blaser, MD, the Frederick H. King Professor of Internal Medicine and professor of microbiology, analyzed data from participants in two National Health and Nutrition Surveys (NHANES III and NHANES 1999-2000) to assess the association between H. pylori and levels of HbA1c.

"Obesity is an established risk factor for diabetes and it is known that high BMI is associated with elevated HbA1c. Separately, the presence of H. pylori is also associated with elevated HbA1c," said Dr. Blaser, who has studied the bacteria for more than 20 years.
